Zanzibar - Covid and required documents?

Hi,
I have 2 question to anyone who recently visited Zanzibar.

1. Is it enough just to fill up this document:

OR two following are required as well?

2. PCR test must be done 96h before entering flight OR before entering Zanzibar?
