Think the loop in the flagout line was too thin if it slipped through. It needs to be a loop with a second line spliced through the loop. I used clear PU tube cover on the safety line. Keeps it from wrapping in or around the cleat. It’s a tight fit where it’s spliced so I don’t worry about sand and not loaded anyway.

here a little screenshot from the described sleeved microhole safety in use.
I see no wear since the last 10 or so autum sessions.
Remark: You see the 90° turn of the flying lines going through the Microholes first and then connected to saftey line on one side and the other to a little sleeve only.
I would say: It works as expected - I like it.
Mostly none bejond regular release checks (under load, to test this).
