Foul hooking. When the chicken loop walks off of the hook just before launching, and then the tension of the kite deforms the donkey dick and unhooks the kite.
Did not watch further than that to see tips. Check your gear before launch.

Make sure the chicken loop doesn't "slip out" while you are walking on the beach without any tension on the lines.
Good tip. I have made that mistake myself once. The donkey dick on the Ozone bar isn't brilliant (at least not the version I got).

Here goes a big error:
DON’T walk around with your chicken loop attached!
Hook it in when you are on the side, 90 degrees to your kite, and ready to launch.
When you stop your session, the helper catches your kite, immediately unhook.
Otherwise you increase your risk of accidents, like the one Damo explains.

Correct great tip
Self launcher here always with one lower palm cupping the chicken loop firmly against the inside of hook
Other arm stretched out to tension the depower line on the upper side of bar
Other advantage is that the hand cupping is just a dew inches from QR in case you need to hit it fast
If i am to help someone to launch i always walk first to them and instruct to palm the CL against the hook
Never trust 100% donkey & chicken sticks

I would agree with that, but sometimes something comes up and you end up taking some steps one way or another.
I do both. I don't connect, but I also double check even if I haven't moved.
Same with jumping. I have done exactly what he describes once probably during a landing. Then for the next jump my kite is gone as I am no longer properly connected. Which now means I automatically check the connection after every jump
