Do I have to make my own? If so what's the best way?
Do they only work with 5th line systems?
Is it best to attach it to the chicken loop?

Almost every kite company offers a suicide leash. If you have a bow kite, put it to the chicken loop. If you have a 5th line than to that and if it's a pulley bar then to the static loop. The best one I've seen so far has been Dakine's leash.

normally every leash can be used as normal 5th line leash and suicide leash.
It just depends where you attach it to.
Pay a bit more for quality to avoid runaway kites, if they break...
